CVS Caremark and Diabetic Supplies

Diabetic supplies are only covered under CVS Caremark pharmacy coverage, and not under the Florida Blue medical plan. This flyer explains the process and how to qualify for the Accu-Chek Blood Glucose Meter.  Please note that the free Accu-Check meters must be ordered by calling CVS Caremark directly at 1-800-588-4456; they cannot be filled through a local retail pharmacy
